Weakness of Health Records Cards

Health records consist of records of patient data is done in health care. The records are very important in the care for patients because of the complete data can provide information in determining the decisions, good treatment, handling, and other medical measures. Physician or dentist is required to make appropriate health records regulations.
Health records in the form of cards have been far from adequate. More often, health records card is too thick, ragged, not organized in a neat, even illegible; progress notes, consultant reports, radiology results and mixed nurse-stirring notes. Health records card is even more confusing, not ease of care; a formidable challenge for anyone who tries to understand what is experienced by patients “. In the use of health records card get a lot of difficulties in the process of its use. There are several things that cause limitations of medical records, namely:
- Contents: It’s hard to find data
- Fragmentation: if each unit or installation of different store medical records for the same person
- To send information: the data need to be copied
- Unable to integrate clinical decision support systems with patient information that has been collected
